Information That You Should Bring With You To Your Consultation

When you meet with a member of our team, a few pieces of information will help us better understand your rights and obligations under your policy. While you may not have all the requested items or cannot find the information, obtain as much of this information as you can and we will help you figure out how to get the rest.

Your Disability Insurance Policy

The most important information to have is a copy of your disability insurance policy or the plan/policy description if you do not have the full policy. The terms stated in the policy will allow us to fully assess and determine what is needed to prove disability. If you do not have a copy of your policy, we can explain how to get it.

Your Medical Information

Additional documents that can help us understand your claim include records from the most recent medical treatment related to your current disabling condition. As we work with you throughout your case, we will collect your full medical history from your treating physicians, your primary care provider, and any special treatment you have received. Imaging and blood tests are especially helpful, as is a list of all doctors that you have seen for the past few years and their contact information.

Past Disability Claims

If you have previously applied for benefits under your disability insurance policy and you were denied, you should bring that paperwork with you so that we can review those prior claims and see how they may impact your current claim.

Personal Journals Or Notes

If you have kept any record of the condition that disabled you, such as a pain journal or record that shows the progression of your condition, that too may help us establish the severity of your condition and how it impacts your functionality. Also, such records make persuasive evidence alongside corroborating medical information.
